Job Description

Alleghany Highlands Community Services provides services and support to individuals in our community with behavioral health challenges and intellectual and developmental disabilities. The Nurse Coordinator works as a team member, providing person-centered active treatment and care for adults with developmental disabilities. The position requires coordination, implementation, documentation, and evaluation of health care services for nine individuals to assure complete integration with other services and the provision of active treatment. The RN is a member of the ICF/IID Services Interdisciplinary team, providing training and direction to staff. The RN works collaboratively with the Supervisor and Medical Director but is also expected to work independently with a high degree of accuracy, initiative, confidentiality, and decision-making skills. The RN will conduct healthcare trainings, medication administration pass audits, and other audits at Merryfield ICF/IID and other locations within AHCS. This position provides training, supervision, monitoring,g and oversight of the skilled nursing services provided to individuals with complex health care needs requiring feeding and medication administration via percutaneous gastrostomy tube.

REQUIREMENTS

• Associate's or Bachelor's Degree in Nursing (Required)
• 1 year of experience in the nursing field (Required)
• Registered Nurse with a current Virginia license (Required)
• Valid Driver's License (required)

RESPONSIBILITIES

• Provides comprehensive health care services to meet regulatory requirements, with the assistance of the consulting Medical Director and the team
• Complete pre-admission, post-admission and annual assessments of each resident's nursing needs
• Develops and directs nursing plan of care for each individual by coordinating with all service providers
• Provides training to AHCS employees including skilled nursing services and medication administration
