El Sol Neighborhood Educational Center is seeking a dedicated Community Health Worker to promote health within the community by empowering individuals to adopt healthier behaviors. The CHW will serve as a trusted advocate, ensuring individuals can effectively communicate with healthcare providers and social service agencies. Acting as a liaison, this role supports the implementation of programs that enhance individual and community well-being, including preventive services and data collection to assess community health needs. This role will contribute to Advancing Equity through Zero Emission Public Transit, Transit Ambassadors, and Workforce Development by addressing community-identified transportation needs, supporting increased access to essential destinations, and facilitating greenhouse gas (GHG) emission reductions. The CHW II will work closely with local transportation agencies to educate the community on zero-emission transit options, advocate for equitable mobility solutions, and assist in workforce development initiatives that support sustainable transportation.

Minimum Requirements

  • High School Diploma or equivalent req; 2-3+ years of related/equivalent experience in a similar position with training/certifications; 80hrs.
  • Bilingual (Spanish/ English; speak, read, and write) preferred.
  • Receive clearance from an FBI background check (Custodian of Records for DOJ).

Qualifications

  • Minimum of 2 years of experience as a CHW, Promotor de Salud, or similar role.
  • Willing to commute within the surrounding work area.
  • Strong knowledge of public health principles and community resources.
  • Excellent interpersonal, communication, and organizational skills.
  • Strong knowledge of public health principles, community resources, and transportation equity.
  • Basic knowledge of Microsoft Office Suite and data collection tools.
  • Frequent local travel to meet with participants, attend community events, and engage in transit-based initiatives.

  • Capable of working/ understanding the needs of a multicultural environment.
  • Strong work ethics, maintaining a professional appearance, and exercising discretion with sensitive information.
  • Ability to perform tasks related to physical activity to include sitting, standing, for long periods of time, caring, bending, moving, and lifting up to 50 lbs.

Responsibilities

  • Serve as a bridge between the community and healthcare providers, ensuring individuals’ voices are heard and needs are met.
  • Actively answer questions, address concerns, and receive feedback from the community regarding public transportation services.
  • In collaboration with Omnitrans, Transit Ambassadors work at bus stops, transit centers, and onboard buses, engaging with the public, answering questions, receiving feedback, and administering surveys.
  • Engage the public, distribute passes, and conduct surveys using tablets while working in the community.
  • Distribute transit passes as survey incentives.
  • Promote zero-emission transit options and workforce development opportunities that align with community-identified transportation needs and greenhouse gas reduction initiatives.
  • Assist individuals in developing personalized strategies to adopt and sustain healthy behaviors.
  • Promote public transit accessibility and rider safety, ensuring individuals, especially those from vulnerable populations, feel comfortable using the system.
  • Serve as a liaison between riders and transit authorities, escalating concerns and advocating for improved services where needed.
  • Provide education on alternative mobility solutions, including electric buses, bike-sharing programs, and pedestrian-friendly infrastructure.
  • Address community-identified transportation barriers, working with partners to develop strategies for increasing mobility and reducing disparities.
  • Participate in outreach and policy discussions to ensure community voices are heard in transportation planning and environmental justice efforts.
  • Collect and report data on community health trends, outreach efforts, and program effectiveness.
  • Support and mentor other Community Health Workers, providing guidance and sharing best practices.
  • Participates in staff meetings, individual supervision, and training related to their work.
  • Other duties as assigned.
