Job Overview:

The Clinical Pharmacist Specialist is responsible for the provision of pharmaceutical care to all patient age groups (including geriatric, adult, adolescent, pediatric, and neonatal patients) with a concentrated focus on Critical Care and Infectious Disease. The Clinical Pharmacist Specialist will have a leading role in Antimicrobial Stewardship, working to optimize clinical outcomes of patients and promote appropriate antimicrobial utilization, while minimizing the unintended consequences of antimicrobial therapy and reducing costs. They are responsible for providing all levels of pharmacy clinical services within specialization in their assigned area, including but not limited to the preparation, dispensing, and monitoring of both the prescribing and effectiveness of all medications and IV admixtures. The job will involve independent decision-making.

The Specialist is responsible for assisting the Director of Pharmacy and Assistant Director of Clinical Pharmacy Services in the coordination and supervision of all clinical pharmacy activities in addition to the quality improvement and quality assurance activities relating to the provision of pharmaceutical care. The Specialist plans, organizes, analyzes, and compiles the facility s medication utilization program(s) as required and assists in the implementation of cost-containment programs with continuous monitoring for compliance and accomplishments.

The Specialist will furnish clinical information to physicians, pharmacy personnel, healthcare practitioners, and patients and will play an integral role in staff education and competency requirements for the department. The Specialist will participate in committees and projects as assigned and other duties as deemed necessary by the Director and Assistant Director.

Qualifications:

Required:

  • Graduate of an ACPE accredited college of Pharmacy with a PharmD

Preferred:

  • Successful completion of a Post Graduate Year 1 residency (PGYI)
  • Board Certification in Pharmacotherapy

Certifications and Licenses Required:

  • NJ Licensed Registered Pharmacist

Essential Functions:

  • Assist in developing and maintaining the antimicrobial formulary, antimicrobial guidelines, and order sets to aid in evidence-based guided care of patients
  • Serve as co-chair of the local Antimicrobial Stewardship Committee and as an active member of the RWJBH Infectious Disease Collaborative and Antimicrobial Stewardship Collaborative
  • Provide cost-effective pharmaceutical care to patients by conducting direct patient care activities using a consistent approach that reflects the philosophy of pharmaceutical care and that is performed with the efficiency and depth of experience characteristic of an experienced infectious disease/AMS pharmacist
  • Conduct daily antimicrobial stewardship activities, including prospective audit and feedback of broad-spectrum antimicrobials
  • Participate in patient care rounds with patient care teams through collaborative interactions with prescribers, nursing staff, clinical pharmacist, and/or pharmacy students
  • Develop and conduct target drug programs and medication use evaluations as needed and report results to the Antimicrobial Stewardship Committee/Pharmacy and Therapeutics Committee to promote process changes to optimize medication utilization
  • Perform as a primary preceptor and mentor for pharmacy students to aid in ensuring successful completion of all rotational requirements as set forth by their perspective schools of pharmacy
  • Provide lectures and other informative activities on infectious disease/AMS topics in to health-care workers
  • Provide clinical support for quality measures through evaluation and analysis of related data
  • Complete a minimum of one quality improvement / research project per calendar year
  • Provide coverage for the centralized pharmacy as determined by need
